Who We Serve

Anchored Behavioral Services is preparing to support adults 18+ who benefit from a structured residential environment, consistent routines, behavioral support, and close coordination among providers, WSCs, families, and the individual’s care team.

A Potential Fit May Include

  • Adults age 18+

  • Individuals with behavioral support needs

  • Individuals who benefit from structure and predictable routines

  • Individuals needing consistent residential supervision and support

  • Individuals who benefit from community participation and meaningful daily activities

  • Situations where strong provider/WSC communication is especially important

Every potential placement is reviewed individually to determine whether Anchored can safely and appropriately meet the person’s needs.

Communication & Coordination

Responsive communication with WSCs and the care team, timely updates when concerns arise, and collaboration around the individual’s changing needs.

Meaningful Daily Life

A focus on community participation, purposeful activities, skill development, relationships, and helping individuals build fulfilling routines beyond the home.

A Simple, Collaborative Referral Process

Our goal is to make it easy for WSCs to explore a potential fit, communicate relevant needs, and understand the next steps without unnecessary barriers.

1. Connect

Contact our team to discuss the individual’s general residential needs and determine whether Anchored may be worth exploring.

2. Initial Fit Discussion

We discuss support needs, behavioral considerations, preferences, and other factors that may affect residential success.

3. Secure Review & Meet

When appropriate, relevant referral information is reviewed through the proper secure process and a meet-and-greet or tour can be coordinated.

4. Placement Coordination

If the individual and Anchored appear to be a good fit, we collaborate with the WSC and care team on transition and placement planning.

Structured & Predictable

Consistent routines, clear expectations, meals, personal-care support, household responsibilities, and dependable staff support help create a stable home environment.

Active & Connected

Residents will be encouraged to participate in appropriate day programs, community activities, recreation, appointments, social opportunities, and other meaningful experiences outside the home.

Individualized Support

Daily routines and supports are shaped around each person’s abilities, needs, goals, preferences, behavioral supports, and approved plan of care.
